A hardcopy of this form must be sent to the Service Corporation and you must receive approval
before
any work is to be done to your property.
Please print this form from your web browser and mail the completed hardcopy with supporting illustrations/drawings to the Service Corporation at the following address:

Your Architectural Change will be decided by the Service Corporation
within 30 days
of the Service Corporation's receipt of a paper copy of this form with supporting illustrations and drawings. All illustrations and drawings will be retained by the S.L.S.C. unless otherwise indicated by you.
Approval of your request by the Service Corporation does not fulfill your other obligations which might be legally required by the Township, County, State or National codes and laws. For example, decks require approval by New Garden Township too. Approval of your request does not imply satisfactory compliance with safety, mechanical, chemical, electrical, or plumbing codes and regulations.
